The form needs to be turned in at least two weeks prior to an event so that we may get the set up scheduled. We always appreciate having the form prior to the two-week deadline.\u00a0<\/span>Please fill out the entire form, include your email address, and return it to the Parish Office or email it to <a href=\"mailto:calendar@stmaryofsorrows.org\" target=\"_blank\" rel=\"noopener\">calendar@stmaryofsorrows.org<\/a>. If yours is an outside organization, also include its Certificate of Insurance.<\/li>\n<li>You may also want to announce your activity in the bulletin.\u00a0 You can do so with the <a href=\"https:\/\/stmaryofsorrows.org\/wp-content\/uploads\/Bulletin_Pulpit-Announcements.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Bulletin Announcement\/Pulpit Announcement Form<\/a><strong>. T<\/strong>his form requires and even longer lead time than two weeks, especially if you would like a full-page color flyer in the bulletin.\u00a0 We have several different options for bulletin arrangements but the sooner we are aware of your needs, the better able we are to serve you and your group or ministry.<\/li>\n<\/ul>\n<div>The Diocese of Arlington now requires forms as well.\u00a0<b>Below is a flowchart to help you navigate both our parish and diocesan requirements<\/b>. These are the Diocesan forms referenced in the flowchart.<\/div>\n<ul>\n<li><a href=\"https:\/\/www.arlingtondiocese.org\/Risk-Management\/Documents\/Renters-Liability\/01_Facility_Use_Agreement_rev2020.pdf\" target=\"_blank\" rel=\"noopener\">Facility Use Agreement<\/a> (include with Parish forms)<\/li>\n<li><a href=\"https:\/\/www.arlingtondiocese.org\/Risk-Management\/Documents\/Renters-Liability\/01_Renters_Liability_Application_rev_07-2022.pdf\" target=\"_blank\" rel=\"noopener\">Renter&#8217;s Liability Insurance Application<\/a> (Mail to Diocese &#8211; address on form) This application must be received by the Office of Risk Management at least 15 days in Advance of the event. Coverage is subject to approval by Waldorf &amp; Associates Agency\u00a0<\/li>\n<\/ul>\n<p>If you plan to have <b>liquor at your event<\/b>, you&#8217;ll need to apply for a license from the VA ABC. <a href=\"https:\/\/www.abc.virginia.gov\/licenses\/get-a-license\/banquet-licenses\" target=\"_blank\" rel=\"noopener\">Here<\/a> is a link where you can do so. It&#8217;s likely you&#8217;ll need a &#8220;Special Event Banquet&#8221; license. Applying on-line is much easier and quicker than other options. If your organization has a non-profit number, the application process will flow much more smoothly.
